The High School has three sport seasons:
· Fall-starts in August
· Winter-starts in November
· Spring-starts in March
The following items are needed to receive sports clearance:
1. Physical exam on or after June 1st of each year. This physical exam will be adequate for all sports for that upcoming school year.
2. Student/parent brings physical exam copy to school nurse or parent may scan and attach in FamilyID for school nurse for review.
3. Parent fills out online registration forms on FamilyID website (). Registration for sports will open on or about 30 days prior to the start of the respective season.
4. If the student has a health issue or injury an additional form or forms must be completed before the student will be cleared for participation. Forms can be downloaded from related files below or picked up in the health office. For example:
-Injury - Physical Education Recommendation Medical form
-Asthma - Asthma Clearance form
-Epi-Pen - Epi-Pen Clearance form
-Any other health issue or past injury - Physical Education Medical Recommendation form
-Med Sheet - Authorization of Medication in School form (if applicable)
5. Once cleared through the school nurse, parents/coaches will receive an email to verify clearance.
